BEFORE YOUR SESSION:
The three forms listed below are REQUIRED to be completed prior to your appointment date and time. Your appointment is not considered confirmed until ALL three of these documents have been electronically submitted OR printed (below) and emailed to firstname.lastname@example.org. Even if you are a repeat client, these three documents need to be completed before EACH session.

3. Customer Satisfaction Survey As a photographer, I want to create beautiful, timeless photos. As a business owner, I want to give my clients a fabulous photography experience from start to finish.
